Comparison of seizure reduction and serum fatty acid levels after receiving the ketogenic and modified Atkins diet

Porta N et al. – A decrease of serum arachidonic acid levels may play a role in the anticonvulsive effects of ketogenic diet (KD) or modified Atkins diet. The KD and modified Atkins diet led to seizure reduction in this small pilot series, with slightly better results after 3 mo with KD, but not after 6 mo.
Methods- Retrospective comparison of the KD and modified Atkins diet in 27 children
- Assessment of serum long chain fatty acid profiles
- After 3 mo, on intent-to-treat analysis, KD was more successful: >50% seizure reduction in 11/17 (65%) with KD vs 2/10 (20%) with modified Atkins diet
- After 6 mo, this difference was no longer significant: 7/17 (41%) vs 2/10 (20%)
- Preventive effect of both diets on occurrence of status epilepticus
- Significant decrease in serum arachidonic acid concentration after 1 and 3 mo of either diet for responders vs non-responders
