DasGupta R et al. – The authors propose using an aminoglycoside with a penicillin for high–risk cases (e.g. endoscopic stone surgery) while low–risk cases (e.g. flexible cystoscopy with no risk factors) might be managed without such prophylaxis. Pathogenic patterns and resistance rates should be monitored regularly.

| I hope our paper encourages others to review their own departmental flora, and consider modifying antibiotic policies accordingly. Also, the importance of Gram positive bacteria in urosepsis will hopefully be appreciated. |
