Clinical features and renal outcome in lupus patients with diffuse crescentic glomerulonephritis

Tang Z et al. – Study reports that the lupus patients with diffuse crescentic glomerulonephritis (DCGN) presented more severe clinical syndromes, which were similar to patients with type II of DCGN. The relative good renal outcomes were observed in those who received intensive immunotherapy.
Methods- Aim was to investigate the clinical features and renal outcomes in lupus pts with DCGN
- 94 DCGN lupus pts; 84 females with a mean age of 27.9 ± 10.7 yrs
- They represented:
- hypertension in 73 cases (77.7%)
- rapidly progressive glomerulonephritis in 62 cases (66.0%)
- nephritic syndrome in 46 cases (48.9%)
- gross hematuria in 35 (37.2%) and
- uremic syndrome needed dialysis therapy 14 cases (14.9%)
- 25 cases received repeated renal biopsy
- Histological examination showed the decrease of active lesions and an increase of chronic lesions
- All pts were followed for >6 mo, and 79 pts for >12 mo
- At first f/u (3 mo), the renal function, proteinuria, and anemia were improved significantly in those who received intensive immunosuppressive therapy
- At last f/u (56.1 ± 18.8 mo), only 4 pts eventually developed the end-stage renal failure and 5 died with normal renal function
