A preliminary result of concurrent chemoradiation with weekly cisplatin in elderly nasopharyngeal carcinoma patients

Ho HC et al. - Weekly cisplatin 30 mg/m2 as concurrent chemoradiation (CCRT) was effective in elderly nasopharyngeal carcinoma (NPC) pts, with acceptable and reversible acute toxicity following CCRT therapy

Methods

  • Retrospective study of efficacy, toxicity, and tolerability of a multi-modal treatment strategy in stage IIB-IV NPC pts age >60 yrs with subsequent systemic adjuvant chemotherapy for systemic control
  • CCRT chemotherapy protocol: 8 weekly doses of cisplatin 30 mg/m2 in an outpatient setting
  • Adjuvant systemic chemotherapy: 4 monthly cycles of cisplatin (20 mg/m2/day) plus 5-fluorouracil (1000 mg/m2/day) for 5 consecutive days
  • Endpoints: treatment outcome, compliance, toxicity

Results
  • 2-yr overall survival, disease-free survival, local control, and distant metastasis-free rate: 87%, 73%, 92%, and 76%, respectively
  • During CCRT, >80% of pts were able to take >6 doses of weekly cisplatin
  • Grade 3 hematologic toxicity in ~50% of pts during adjuvant therapy requiring treatment modification or cessation of further adjuvant therapy
