Wynn KK et al. – The authors show that large monoclonal/oligoclonal repertoire expansions are associated with the loss of HCMV–specific T–cell function observed in SOT patients undergoing symptomatic viral infection or recrudescence, whereas SOT recipients who maintain peripheral blood TRBV repertoire diversity and functional antigen–specific T–cell responses can resist clinical symptomatic disease in spite of high levels of viral load.

Acquisition of primary herpes virus infection or recrudescence of an existing infection is commonly observed in solid organ transplant patients. However, only a small proportion of these patients progress to develop clinical symptoms. In this manuscript we demonstrate that symptomatic manifestaion of EBV and/or CMV infection in co-incident with dramatic change in peripheral blood T cell repertoire and loss of T cell function. As these patients recover from symptomatic infection the TcR repertoitr reverts back to polyclonal profile. We beleive that the long-term inplication of this data is that in future a combination of viral DNA load, T cell function and TcR repertoire analysis may be helpful in predicting which patients will progress to develop the symptomatic disease and thus reduce the use of anti-viral drugs.
