Nielson DL et al. – The authors present cases with severe infections leading to gangrene and ischemia, which were improved following surgical management with consequent improvement in tissue viability. In these cases, the authors demonstrate that immediate treatment of the wound despite the delayed presentation of the patients resulted in limb salvage with much less soft–tissue loss than expected before treatment.

What I wanted to say in the article but could not get past the editors is that if you have an ischemic wound, you can still debride if you only remove the tissue that is already dead. People get into trouble when they injure viable tissue. Wounds of this caliber actually show remarkable improvement when only the dead tissue is removed. Here in the States, it is common to not touch the wound until Vascular has improved the arterial status.
