Rogers BA et al. - In an Australian setting, methicillin resistant Staphylococcus aureus (MRSA) endocarditis endocarditis was primarily nosocomial or healthcare associated. Common characteristics were older pts with multiple co-morbidities. Despite high rates of combination antibiotic therapy and surgery, mortality was very high.
Methods- Retrospective case series of MRSA endocarditis pts between 1991-2006
- Subjects: 18 males (67%); median age: 64 yrs
- Study of the characteristics, management, and outcomes of pts with MRSA endocarditis in an Australian hospital and trends in this group over 16 yrs
- Infection related to prosthetic valve or annular ring in 10 pts (37%)
- Most common comorbidities: diabetes mellitus in 8 pts (30%); malignancy in 8 (30%)
- Nosocomial acquisition in 16 pts (59%); non-nosocomial healthcare-associated acquisition in 10 (37%); community acquisition in 1 (4%)
- Management: single antimicrobial agent in 5 pts (19%); combination antimicrobial therapy in 22 (81%)
- Surgery: 16 pts (59%)
- Mortality: 66%
- Over 16 yrs, increased non-nosocomial acquisition and presentations to non-tertiary hospitals
- No clear improvement in survival over 16 yrs
