McCormick J – Massachusetts General Hospital, like many other health care facilities, had a number of anesthesia cases that were not billable because of poor documentation. There were missing compliance statements, inaccurate listings of when anesthesia was administered, unsigned records, and other errors. And, of course, claims with overt errors or missing information can't be sent to an insurance company for reimbursement.
