Harrison A et al. - It argues that some features of central government policy have been important – such as the use of targets – others, such as the introduction of new private sector capacity have not. It also shows that changes at local level have been critical to achieving the recorded improvements, but the precise impact of these is hard to identify.

A key point in our article is that waiting times measures must include all waiting times- not simply those after the decision to treat has been made. Unless all waits are included, managers can fool themselves into believing that the situation has improved when in fact waits have been shifted from one place to another. Less helpfully we found it hard to define how the NHS success was achieved, apart from the importance of government defined targets. One or two things are clear: any attempt to reduce waiting times must be sustained for a long period.And care pathways must be redesigned and shortened.
