| Today, the most serious and costly complication of the treatment of hemophilia A that remains to be overcome is the development of inhibitory antibodies (FVIII inhibitors) to FVIII replacement therapy. In this paper we present a new early prophylaxis regimen that is shown to reduce the inhibitor risk in previously untreated children with severe hemophilia A by avoiding immunological danger signals within the first FVIII exposure days. Although our results should be interpreted as hypothesis generating and would need to be confirmed in a larger prospective clinical study we think that they are very important for hemophilia care givers in order to develop safer hemophilia treatment regimens that can reduce the risk of inhibitor development in previously untreated patients. Another benefit of the new prophylaxis regimen is that the proposed once weekly administration is simpler for parents, requiring only one visit to the hemophilia centre each week, so that compliance with and control of the therapy is easier to achieve. There would be also a pharmacoeconomic benefit in that the proposed lower doses and less frequent administration can allow considerable cost savings compared with standard prophylactic regimens. |
