A phase 2 clinical trial of nab-paclitaxel in previously treated and chemotherapy-naive patients with metastatic melanoma

Hersh EM et al. – nab-Paclitaxel was found to be well tolerated and demonstrated activity in both previously treated and chemotherapy-naive patients with MM. The response rate, PFS, and survival compared favorably with current standard dacarbazine therapy and combination therapies for melanoma.
Methods- Patients with histologically or cytologically confirmed, measurable MM enrolled
- nab-Paclitaxel administered iv weekly for 3 of 4 weeks at dose of 100 mg/m2 (in previously treated patients) or 150 mg/m2 (in chemotherapy-naive patients)
- 37 patients treated in each cohort
- Response rate was 2.7% in previously treated cohort and 21.6% in chemotherapy-naive cohort
- Response plus stable disease rate 37.8% and 48.6% in previously treated and chemotherapy-naive cohorts, respectively
- Median PFS 3.5 months and 4.5 months, and median survival 12.1 months and 9.6 months, respectively
- Probability of being alive and free of disease progression at 6 months 27% for previously treated cohort and 34% for chemotherapy-naive cohort
- Probability of surviving 1 year 49% and 41%, respectively, for previously treated and chemotherapy-naive cohorts
- Approximately 78% of previously treated patients and 49% of chemotherapy-naive patients treated without dose reduction
- 8 (22%) chemotherapy-naive patients discontinued therapy due to toxicities
- Drug-related toxicities included grade 3 to 4 (graded according to National Cancer Institute Common Terminology Criteria for Adverse Events [version 3.0]) neuropathy, alopecia, neutropenia, and fatigue
