This study was conducted in the context of the UK National Health Service. This means that there was no charge to any of the patients in having diagnostic tests or treatment. Nevertheless there was a consistent disadvantage to patients who live in poorer areas in gaining access. We believe that this disadvantage exerts its effects on events before the patient comes to the attention of a specialist. So, the concern is greatest for residents of deprived areas who live at greatest distances from the relevant healthcare facility.
