Dose intensity and hematologic toxicity in older breast cancer patients receiving systemic chemotherapy

Shayne M et al. – No differences with regard to chemotherapy-related hematologic toxicities between older and younger breast cancer patients in this large prospective observational study was recorded; partly attributed to more frequent reductions in relative dose intensity (RDI) and less frequent utilization of anthracyclines among older patients.
Methods- An evaluation of patient and treatment characteristics that contributed to hematologic toxicity in older BCa pts
- Data collected on 1224 pts with stage I through III BCa, of whom 207 were ≥65 yrs
- Primary outcome measures: anemia, thrombocytopenia, severe neutropenia, febrile neutropenia, and both planned and actual RDI
- Comparisons between older and younger pts regarding hematologic toxicity and reductions in RDI based on univariate and multivariate logistic regression analyses
- Neutropenic complication rates were similar in older and younger pts
- No differences in rates of anemia or thrombocytopenia
- Approx. 34.0% of the older pts received RDI <85% vs 20.0% among younger pts
- Fewer older pts received anthracycline-based chemotherapy vs younger pts
- Fewer older pts received prophylactic white blood cell colony-stimulating factor vs younger pts
