Proton beam therapy for hepatocellular carcinoma

Nakayama H et al. - In a study to examine one experience treating hepatocellular carcinoma (HCC) pts with proton beam therapy, it was shown that proton beam therapy can be both safe and effective for the treatment of pts with HCC. Consideration of proton beam therapy in pts for whom other treatment options are risky or contraindicated is strongly recommended.
Methods- 333 pts with HCC were treated with proton beam therapy; 318 pts were included in the study.
- Total dose delivered and fractionation scheme were determined by protocols that varied based on location of tumor.
- Survival rates and prognostic factors were assessed.
- Overall actuarial survival rates at 1-yr, 3-yrs, and 5-yrs were 89.5%, 64.7%, and 44.6%, respectively.
- Child-Pugh liver function, T stage, performance status, and planning target volume significantly impacted survival.
- 3-yr and 5-yr survival rates were 69.1% and 55.9%, respectively, for pts with Child-Pugh A disease and 51.9% and 44.5%, respectively, for pts with Child-Pugh B disease.
- Actuarial survival rates of pts with Child-Pugh class A were statistically different between groups of planned target volume 125 mL and >125 mL.
