McIntosh A et al. – In this initial experience with accelerated IMRT plus capecitabine for patients who had large HCC lesions, the results demonstrated acceptable toxicity with promising local control. The relatively low acute and late toxicity observed with this program suggested that dose intensification can be incorporated into the treatment regimen if needed.

This combination treatment builds onto approaches other radiation oncology groups in the Orient, Europe, and North America have used recently for the treatment of large HCC tumors that are non operable. In regard to tumor size, our Childs-Pugh class A patient's tumors were nearly twice the diameter of those treated in the recently published trial by Llovet, et. al., using systemic therapy with the kinase inhibitor, Nexevar, while our patients appear to enjoy an overall survival far exceeding that obtained with the systemic biologic therapy alone. One of the exciting challenges facing hepato-oncologists will be to find ways to integrate IMRT with radiation sensitization with these newer therapies that clearly mark exciting choices for these patients.
