Paediatric high and low grade glioma: The impact of tumour biology on current and future therapy

Hargrave D – Review discusses the current therapies available for pediatric brain tumors, and highlights the need of targeted therapies. Main points of discussion are:
- Most common type of paediatric brain tumour range from benign low grade gliomas to aggressive brainstem gliomas
- Current therapies: neurosurgery, radiotherapy, chemotherapy or combination of three
- Increasing knowledge of glioma biology is starting to impact on drug development towards targeted therapies:
- An activated BRAF/MAPK/ERK pathway in the majority of cases of pilocytic astrocytoma is reported
- Targeted therapies against VEGFR are already playing a role in the management of glioblastoma
- It is likely that high grade gliomas in children and adults share common aberrant molecular pathways
- Selection for specific targeted therapy will require individual case by case testing for target presence to maximise the efficacy of molecular therapy
