Tsiatis AC et al. – Detection of nuclear c–Myc in leiomyosarcoma predicts decreased overall and metastasis–free survival, independent of standard prognostic variables, tumor size, histological grade, and TNM stage. The expression of this oncoprotein may represent a useful prognostic marker and potential therapeutic target in leiomyosarcoma.

Making treatment decisions for patients with leiomyosarcoma is complicated by the clinical heterogeneity that exists within this patient population. Although clinical and pathologic parameters are used to define poor, intermediate, and good prognosis groups, these definitions are inadequate, as patient outcomes are diverse and difficult to predict even within these prognostic groups. Our results suggest that c-Myc may be an important factor in further defining the prognosis for individual patients with this rare tumor type. More importantly, these results implicate c-Myc as a pharmaco-therapeutic oncogenic target. Based on these preliminary data, collaborative studies, including clinical trials, may now be designed to assess the clinical utility of anti-c Myc pharmacotherapeutics currently under investigation or in development. It is our hope that this study will lead to the optimization of current treatment protocols and the design of novel therapeutic regimens for patients with c Myc-positive soft tissue leiomyosarcoma and provide our patients with much needed hope for substantial, efficacious treatment options.
