Preoperative evaluation of synchronous colorectal cancer using MR colonography

Achiam MP et al. - Study demonstrates that the feasibility and potential gain of preoperative magnetic resonance colonography (MRC) in patients with sigmoid colon cancer or rectal cancer.
Methods- Aim was to determine the feasibility of performing MRC in pts:
- Who were not offered a full conventional colonoscopy or
- In whom full conventional colonoscopy was not possible
- In a 13-mo period, 47 pts diagnosed with rectal or sigmoid colon cancer scheduled for operation were enrolled
- MRC was performed with bowel purgation either:
- night before surgery or
- as ambulatory MRC the week before surgery
- Full MRC was performed in 98% of the pts
- In 4 pts, 12 synchronous lesions (1 cancer, 2 plaques of carcinosis, and 9 adenomas) were found
- 1 flat adenoma and 5 small polyps were missed by MRC and perioperative palpation but found on postoperative colonoscopy
- Findings resulted in altered operative strategies in 3 pts
