Radiation therapy in addition to gross total resection of retroperitoneal sarcoma results in prolonged survival: results from a single institutional study

Zagar TM et al. - In a study to assess the survival advantage of radiation therapy (RT) in addition to gross total resection of retroperitoneal sarcoma (RPS), it was concluded that compared with surgery alone, neoadjuvant or adjuvant RT offers these pts an excellent chance for long-term locoregional control (LRC), distant disease free survival (DDFS), and overall survival (OS). The integration of modern treatment planning for external beam radiation therapy and intraoperative radiation therapy (IORT) allows for higher doses to be delivered with acceptable toxicities.
Methods- 31 pts with RPS treated with gross total resection and RT make up this retrospective analysis.
- 19 were treated preoperatively and 12 postoperatively (median dose, 59.4 Gy); 16 also received IORT (median dose, 11 Gy).
- Pts were followed with stringent regimens, including frequent CT scans of the chest, abdomen, and pelvis.
- With a median follow-up of 19 mos, 2-yr OS rate is 70%.
- 2-yr locoregional control (LRC) rate is 77%.
- 2-yr DDFS rate is 70%.
- There were no differences in radiation-related acute and late toxicities among pts treated pre- versus postoperatively, whether with or without IORT.
