Predictive value of 18F-FDG PET and somatostatin receptor scintigraphy in patients with metastatic endocrine tumors

Garin E et al. – Findings show that 18F-FDG PET exhibits excellent predictive values for early tumor progression; 18F-FDG PET and somatostatin receptor scintigraphy (SRS) results correlate with progression-free survival and overall survival even for histologically low-grade tumors.
Methods- Study to investigate whether 18F-FDG PET and SRS can predict early disease progression and patient survival
- A prospective study of pts with metastatic neuroendocrine tumor diagnosed between Sep 2003 and Jan 2006
- CT, SRS, and 18F-FDG PET were performed and histologic data reviewed
- CT was repeated every 3 mo to assess the risk of early progressive disease, PFS, and OS
- 38 pts; mean age, 60 ± 15 yrs; 4 pts had a high-grade and 34 a low-grade tumor
- Results of 18F-FDG PET and SRS were positive in 15 and 27 pts
- The 2-yr OS and PFS were 73% and 45%; 16 pts had early progressive disease.
- Most 18 F-FDG PET-positive pts and SRS-negative pts had early progressive disease
- 18 F-FDG PET gave excellent negative and positive predictive values of 91% and 93%
- 18 F-FDG PET results correlated with PFS and OS even when only low-grade tumors were considered
- SRS was associated with PFS and OS
- Multivariate analysis: only 18F-FDG PET was predictive of PFS
