Gene expression profiling of plasma cells at myeloma relapse from total therapy 2 predicts subsequent survival

Nair B et al. - Study implies that based on its post-relapse survival (PRS) predictive power, gene expression profiling (GEP) analysis should be an integral part of new agent trials in search of better therapy for high-risk myeloma.

Methods

Aim was to report on prognostic implications for PRS of a GEP-defined risk score at relapse
120 myeloma pts previously enrolled in Total Therapy 2 were available

Results

Among the 71 pts with additional GEP baseline information
3-yr PRS was 71% in 40 pts with low-risk present both at baseline and relapse vs 17% in 28 pts with relapse high-risk
Multivariate analysis: relapse parameters available in 104 pts, high-risk conferred short PRS, while relapse Hyperdiploidy predicted long PRS
In case the initial PR lasted < 2 yrs, relapse low-risk identified 26 pts with superior 3-yr PRS of 61% vs 9% among 32 with relapse high-risk
