The utility of radioiodine scans prior to 131I ablation in patients with well-differentiated thyroid cancer

Nostrand DV et al. - Pre-ablation radioiodine (RAI) scans demonstrate a significant number of findings that may alter the management of patients with well-differentiated thyroid cancer prior to 131I ablation.
Methods- Study to evaluate the utility of RAI scans prior to 131I ablation in pts with well-differentiated thyroid cancer
- All RAI scans performed were reviewed
- Pts excluded who were suspected of having:
- loco-regional disease
- distant metastases
- and/or physiological uptake
- RAI scans performed either 24h or 48h after dosing with 37–148 MBq of 131I
- One reviewer blindly evaluated each set of scans using 6 criteria:
- No. of foci of RAI uptake in thyroid bed/neck
- Location(s) of these foci
- Size of the largest foci
- Percent uptake
- Uptake suggestive of distant metastases
- Significant altered biodistribution
- 53% of pts had findings on the RAI scans that might have altered the pt's management prior to their 131I ablation
- The data grouped by the criteria noted above were:
- 12% with ≥6 foci suggesting local metastases and 6% with no focal uptake
- 14% with suggestion of lymph node metastases
- 1.1% with ≥1 focus lobe
- 8% with ≥15% uptake
- 4% with distant metastases
- 16% demonstrating altered distribution with 6% breast, 3% salivary, 10% GI, and 0.3% urinary bladder
