Burstein HJ et al. – Bevacizumab and vinorelbine are well tolerated and effective as treatment for refractory breast cancer. Plasma vascular endothelial growth factor therapy (VEGF) warrants further evaluation as a prognostic marker for treatment outcome in advanced breast cancer pts receiving anti-VEGF therapy.
Methods- Study of the efficacy and safety of the humanized monoclonal anti-VEGF antibody, bevacizumab, and vinorelbine as treatment for refractory breast cancer
- Investigation of the role of plasma VEGF as a predictor of treatment outcome
- Eligibility: 1-2 prior chemotherapy regimens for metastatic breast cancer or recurrence within 12 mo of adjuvant therapy with measurable disease and adequate end-organ function
- Administration of bevacizumab 10 mg/kg every 2 wks, and vinorelbine each week, until tumor progression or prohibitive toxicity
- Measurement of plasma VEGF at baseline
- Of 56 women treated on protocol, 34% response rate
- Median time to progression: 5.5 mo
- Activity regardless of tumor hormone receptor status or type/extent of prior chemotherapy
- Side effects: uncomplicated neutropenia, hypertension, nasal congestion/epistaxis, neuropathy
- Post-surgical impaired wound healing in 3 pts
- Rare thrombosis or clinically significant proteinuria
- Lower levels baseline VEGF associated with longer time to progression
