Nga M-E et al. – Combination of positive HBME-1 (luminal/membranous) and CK 19 (cytoplasmic) staining on cell blocks of thyroid cytologic specimens is highly discriminatory in the diagnostic workup for papillary thyroid carcinoma

Methods

  • Study of the value of immunostain for HBME-1 and CK19 expression in cell blocks prepared from needle aspiration specimens from 22 thyroid nodules with follow-up histology

Results
  • Both CK19 and HBME-1 were strongly positive in all 9 cases of papillary thyroid carcinoma, the latter showing distinct luminal accentuation
  • Of non-papillary carcinomas, none were positive for both HBME-1 and CK19
  • Of 6 Hurthle cell neoplasms, 2 were positive for CK19 but all were negative for HBME-1
  • Of 9 goitrous nodules, 1 was strongly positive for HBME-1 with luminal/membranous staining but negative for CK19
  • Sensitivity, specificity, and positive predictive value in distinguishing between papillary thyroid carcinoma and goitrous nodules/Hurthle cell neoplasms: HBME-1, 100%, 92.9%, 0.9; combination HBME-1 and CK19, 100%, 100%, and 1, respectively
