Vazquez VDL et al. – > Squamous cell carcinoma (SCC) pts with local advanced tumors are at increased risk of lymph node metastasis associated to previous lesions at tumor site, and prognosis is worse for T4 tumors; lymph node recurrences in N0 pts, once treated, did not affect survival and thus close follow up and prompt treatment of metastasis for these pts is preferred to elective lymphadenectomy or sentinel node mapping Methods
Retrospective study of risk factors for lymph node metastasis and outcome in 57 pts with locally advanced SCC of trunk and extremities
Review of clinical variables: age, gender, ethnicity, previously injured skin (burns, scars, ulcers), patient origin (rural or urban), anatomic site and treatment
Results
Previous skin lesions in 15 pts: 36 T3 tumors; 21 T4; 46 N0, 11 N1
Lymph node metastasis during follow up in 11 N0 pts
On univariate analysis, presence previous skin lesions (ulcers; scars) was a risk factor for lymph node metastasis
Better survival associated with T3 classification
Survival similar in N0 pts with lymph node metastasis during follow up (on lymphadenectomy) vs pts without lymph node recurrence
