Lu JJ et al. – A higher biologically effective dose is possible with accelerated concomitant boost (C-Boost) radiotherapy regimen vs conventional radiation schedules; local control after C-Boost radiation is for stage II nasopharyngeal carcinoma (NPC) pts; further study to confirm efficacy is needed Methods
Prospective phase II clinical trial of the outcome of 47 stage II NPC pts treated with external beam radiotherapy delivered by a C-Boost schedule
Primary tumor and clinically involved nodes: total dose of 72 Gy in 42 fractions
C-Boost for gross disease: 18 Gy in 12 fractions starting on day 19, delivered ≥6 h after first dose
Assessment for response, survival, and toxicity
Results
At median follow-up of 30 mo, 4 pts had local recurrence only, 2 had persistent neck nodal disease or recurrence, and 1 had both locoregional recurrences
Distant metastases in 5 pts with/without locoregional recurrence
Mortality due to: nasopharyngeal cancer in 5 pts; effects of distant metastases in 4 pts; progressive local disease in 1 pt
