Effect of preoperative chemotherapy on liver resection for colorectal liver metastases

Mehtaa NN et al. - Oxaliplatin (Ox)-based preoperative chemotherapy is associated with vascular alterations in liver parenchyma without significant increase of the risk of steatosis or postoperative morbidity/mortality

Methods

  • Prospective study of effects of preoperative chemotherapy on liver parenchyma morphology and morbidity/mortality after liver resection for colorectal liver metastases (CLM)
  • Review of 3 grps of 173 pts who had liver resection for CLM
  • Grp A (70 pts): preoperative Ox; grp B (60): other chemotherapeutic agents (OC); grp C (43): surgery alone without chemotherapy (SA)
  • Comparison of blood transfusion, hospital stay, operative procedure, peak postoperative bilirubin levels, complications, resected liver histopathology

Results
  • Intra-operative blood transfusion and biliary complications higher with Ox-based chemotherapy
  • Sinusoidal dilatation (mild) with Ox-based chemotherapy
  • Steatosis: similar among grps
  • Postoperative mortality: grp A, 2 pts; B, 1; C, 4
