Prognosticators of survival in differentiated thyroid carcinoma

Parikh HK et al. – Follicular carcinoma incidence is significantly high in india (48%), with 65% of study pts in a high-risk grp and 53% incidence of contralateral lobe disease after thyroidectomy; thus more aggressive treatment policy is warranted with total thyroidectomy as the treatment of choice

Methods

  • Retrospective study of the impact of pt and tumor factors on survival
  • Development of a simple risk-grp staging system (high vs low risk) to predict survival in pts with differentiated thyroid carcinomas
  • Review of 417 pts: follicular carcinomas, 198; papillary carcinomas, 219

Results
  • Factors with significant impact on survival: age ≤ 40 yrs, tumor size <5 cm, extrrathyroidal spread, and distant metastasis; impact remained for subset analysis of follicular and papillary carcinomas separately
  • Significant survival advantage for low-risk pts
