Strnad P et al. – No parametrial involvement (PI) was noted in early cervical cancer pts with negative sentinel lymph nodes (SLNs); however, 28% of women with positive SLN had PI; results were highly significant, suggesting that radical removal of parametrium in SLN-negative pts is questionable

Methods

  • Prospective study of incidence and distribution of pelvic lymph node (LN) involvement, SLN involvement, and pathologic PI in stage Ia2 and small Ib1 cervical cancer
  • PI defined as positive parametrial LN or discontinuous malignant cells in parametrium
  • After radical abdominal hysterectomy, 158 pts stratified into 2 grps based on tumor size
  • Grp 1 (91 pts): tumors <20 mm; <50% stromal invasion
  • Grp 2 (67 pts): tumors 20-30 mm; infiltration not more than 2/3 of cervical stroma

Results
  • Grp 1: positive SLN in 11 (12.1%) pts; positive PI in 3 (27.3%) of these pts; no PI in 80 pts with negative SLN
  • Grp 2: positive SLN in 14 (20.9%) pts; PI in 4 (28.6%) of these pts; no PI in 53 pts with negative SLN
