Electrophysiologic Characteristics of Wide QRS Complexes during Pharmacologic Termination of Sustained Supraventricular Tachycardias with Verapamil and Adenosine: Observations from Electrophysiologic Study

Türkoglu C et al. – Noncatheter induced, five different appearance patterns and four distinct morphologies of ventricular origin were observed. Most of them do not directly terminate tachycardia, but are associated with its termination and are not observed in ongoing tachycardia.

| This is the only one study, evaluating intracardiac recordings during pharmacological termination of supraventricular tachycardias and investigating the frequency and mechanism of wide QRS compleces observed in this setting. All previuos reports regarding wide QRS complexes during termination of SVT were presenting data from only surface electrocardiographic records. Here we found that all of these wide QRS complexes are of ventricular origin. They are terminating tachycardia themselves but are associated with its termination with positive predictive value of 100%. In addition we found that thier presence may depend on type of tachycardia and parmacologic agent used to terminate it. We have also described here five appearance pattern and four distinct QRS morphologies of these wide QRS complexes. Mechanism underlying these electrocardiographic fenomena are suggested as abnormal automaticity and/or triggered activity. |
