Uninterrupted Warfarin for Periprocedural Anticoagulation in Catheter Ablation of Typical Atrial Flutter: A Safe and Cost-Effective Strategy

Finlay M et al. – Catheter ablation of typical AFL without interruption of warfarin appears safer and more cost–effective than periprocedural conversion to LMWH. It could be used as a routine anticoagulation strategy for the ablation of right–sided arrhythmias.

| We report our data regarding bleeding complications from catheter ablation for atrial flutter whilst using either low molecular weight heparin around the time of the procedure, or continuing patients on warfarin. There were substantially fewer bleeding complications in the group of patients with uninterrupted warfarin, but we found this group required more TOEs prior to ablation. This was probably because the warfarin group had undergone more anticoagulation checks than the LMWH group, and so more instances of sub-therapeutic INRs were discovered. We have adopted an uninterrupted warfarinisation strategy as our routine periprocedural strategy for right sided arrhythmias, and suggest this as a preferred strategy. |
