Novack V et al. – Among unselected pts undergoing percutaneous coronary intervention (PCI), adjusted rates of both ischemic complications and clinically important restenosis were similar for sirolimus-eluting (SES) or paclitaxel-eluting stents (PES). The unexpected finding that target lesion revascularization (TLR) was influenced by site characteristics suggests that correlation between TLR and angiographic restenosis might be weaker than previously reported.
Methods- Review of data from Evaluation of Drug Eluting Stents and Ischemic Events (EVENT) registry
- Comparison of in-hospital and 1-yr outcomes among unselected pts undergoing nonemergent PCI with either SES or PES implantation
- Primary endpoints: cardiac death or myocardial infarction (MI)
- For 6,035 pts at 47 US centers between July 2004-June 2006, PCI with either SES (3,443 pts) or PES (2,592 pts)
- Baseline clinical and angiographic characteristics similar for both stent types
- At 1-yr, no differences in primary endpoint between the SES and PES groups (9.1% vs 10.0%) or in any individual endpoints: cardiac death, nonfatal MI, or stent thrombosis
- In unadjusted analyses, TLR slightly more common with SES vs PES (4.4% vs 3.3%)
- No difference on adjustment for baseline characteristics and site-related factors
