A meta-analysis of 3,773 patients treated with percutaneous coronary intervention or surgery for unprotected left main coronary artery stenosis
JACC - Journal of the American College of Cardiology: Cardiovascular Interventions, 08/24/09
Naik H et al. – Findings show no difference in mortality or major adverse cardiovascular or cerebrovascular events, for up to 3 yrs, between percutaneous coronary intervention (PCI) and coronary artery bypass grafting (CABG) for treatment of unprotected left main coronary artery (ULMCA) stenosis. However, PCI pts had a significantly higher risk of target vessel revascularization. In select pts with ULMCA stenosis, PCI is emerging as an acceptable option.
Methods- Study to understand the total weight of evidence regarding outcomes in CABG vs PCI in ULMCA stenosis
- Databases search for clinical studies that reported outcomes after PCI and CABG for treatment of ULMCA stenosis
- Identification of 10 studies with a total of 3,773 pts
- Meta-analysis: death, myocardial infarction, and stroke (major adverse cardiovascular or cerebrovascular events) similar in PCI- and CABG-treated pts at 1, 2, and 3 yrs
- Target vessel revascularization significantly higher in PCI group at 1, 2, and 3 yrs
- No difference in mortality in PCI- vs CABG-treated pts at 1, 2, and 3 yrs
