Podoleanu C et al. – In pts affected by delayed orthostatic hypotension (OH), progressive decrease of systolic blood pressure (SBP) is associated with progressive decrease of total peripheral resistance (TPR), whereas cardiac output (CO) and stroke volume (SV) show little variation; the compensatory increase in heart rate (HR) is insufficient to compensate for the decline in blood pressure.
Methods- Study of hemodynamic mechanisms underlying the syndrome of delayed OH and hemodynamic effect of treatment with leg compression bandages
- Non-invasive monitoring of SBP, HR, CO, SV, and TPR during tilt testing in pts affected by delayed OH vs age- and sex-matched controls
- Assessment of 13 pts age 68 +/-14 yrs and 9 controls age 68 +/- 8 yrs
- Immediately after tilt-up, SBP progressively decreased in pts but not in controls
- Symptoms developed at 100 +/-7 mmHg
- At test end, SBP significantly lower in pt vs controls
- TPR progressively decreased in pts but not in controls
- SV and CO did not change significantly
- HR increased progressively in pts until test end and remained unchanged in controls
- Active treatment with compression bandages avoided decrease in SBP and TPR, whereas CO showed minimal variation
