Assessment of cardiac resynchronization therapy response

Cattadoria G et al. – Echocardiography and CPET are complementary for evaluation of cardiac resynchronization therapy (CRT), but not in less severe chronic heart failure (CHF), for which the role of CPET remains uncertain.
Methods- Study of how to assess CRT efficacy in CHF through clinical, echocardiographic and exercise analysis
- Analysis of 41 stable CHF (NYHA III) pts
- Pt characteristics: left bundle-branch-block, ejection fraction <35%, left-ventricular dissynchrony (by tissue Doppler), peak oxygen consumption (VO2) <16 mL/kg/min, suitable cardiac vein (by multislice computed tomography), no anemia or kidney failure
- Evaluation before/after (7±3 mo) CRT
- Mortality: 2 pts
- CRT responders to none of evaluated criteria: 19.5%
- Best agreement (90%) with clinical response by using presence/absence of either left-ventricular systolic volume or peakVO2 response
- In less severe CHF (peakVO2 12–16 mL/kg/min), peakVO2 and work-load did not change after CRT, despite echocardiographic, ventilation/carbon dioxide relationship and clinical improvement
