Holmqvist F et al. – Results are the first to describe changes in P-wave morphology in pts prior to atrial fibrillation (AF) development; findings indicate that abnormal interatrial conduction may play a role in AF development in pts with prior myocardial infarction and congestive heart failure.
Methods- Study of association between P-wave morphology and future AF development in Multicenter Automatic Defibrillator Trial II (MADIT II) population
- Comparison of MADIT-II pts without AF history with baseline sinus rhythm who developed AF during study (Pre-AF) vs matched controls without AF development (No-AF)
- Mean 20-mo follow up
- Baseline 10-min high-resolution bipolar ECG recording
- Analysis of signal-averaged P waves for orthogonal P-wave morphology, P-wave duration, and RMS20
- P-wave morphology then classified as 1 of 3 predefined types by automated algorithm
- 30 pts (age 68 ± 7 yrs) who developed AF during MADIT-II compared with 60 pts (age 68 ± 8 yrs) who did not
- Not significant differences for P-wave duration and RMS20 in Pre-AF group vs No-AF group
- Distribution of P-wave morphologies shifted away from Type 1 in Pre-AF group vsNo-AF group
