Park JH et al. – Morphologically remodeled left atrial (LA) has low endocardial voltage, and enlargement of anterior LA (ALA), is more significant in electroanatomically remodeled LA. The disproportional enlargement of ALA was observed more often in paroxysmal atrial fibrillation (PAF) pts with recurrence after ablation vs pts without recurrence.
Methods- Study of whether LA morphology, based on embryological origin, affects outcome of RFCA
- Analysis of 3D computed tomographic (CT) images of LA in 70 AF pts (54 males, 55.6 ± 10.5 yrs, PAF:persistent AF (PeAF) = 32:38) who had RFCA
- Each LA image divided as venous atrium (VA), anterior LA (ALA), LA appendage (LAA), and both antrum
- Absolute and relative volumes calculated
- Lengths of linear ablation sites measured
- In pts with mean LA voltage ≤2 mV, larger LA volume, especially ALA, vs pts with LA voltage >2mV
- Total LA volume significantly larger and LAA voltages (and conduction velocities lower for PeAF vs PAF pts
- In pts with recurrence, LA volume generally larger vs pts without recurrence
- In PAF pts with recurrence, relative volume of ALA significantly larger vs pts without recurrence
