Atrial overdrive pacing in sleep apnoea: A meta-analysis

Baranchuk A et al. – Although atrial overdrive pacing (AOP) is associated with a statistically significant reduction in apnea–hypopnea index (AHI), the magnitude of this benefit is small and likely not of clinical significance. AOP should not be universally indicated in sleep apnea (SA) pts unless they have a conventional indication for cardiac pacing.
Methods- Systematic review of all randomized clinical trial data evaluating AOP for treatment of SA
- Meta-analysis to estimate true effect of AOP on SA
- Literature review for all reports on AOP effect for SA treatment
- Study eligibility: randomized, controlled, trials; use of AHI to determine SA severity
- In 10 studies, 175 pts with intermediate to severe SA
- Overall, AOP reduced AHI by –4.65 episodes/hr
- In comparison, studies with continuous positive airway pressure (CPAP) arm found a greater reduction in AHI with CPAP: –46.3 episodes/hr
