Incremental value of copeptin for rapid rule out of acute myocardial infarction

Reichlin T et al. – Additional use of copeptin may allow a rapid and reliable rule out of acute myocardial infarction (AMI) already at presentation and thus obviate the need for prolonged monitoring and serial blood sampling in most pts.
Methods- Study of the incremental value of copeptin for rapid rule out of AMI
- Subjects: 487 consecutive emergency department pts with symptoms suggestive of AMI
- Measurement of levels of copeptin at presentation by a novel sandwich immunoluminometric assay in blinded fashion
- Final diagnosis by 2 independent cardiologists using all available data
- Adjudicated final diagnosis: AMI in 81 pts (17%)
- Copeptin levels significantly higher in AMI pts vs pts with other diagnoses
- Combination of troponin T and copeptin at initial presentation resulted in area under the receiver-operating characteristic curve of 0.97; significantly higher than 0.86 for troponin T alone
- Copeptin level <14 pmol/L in combination with troponin T ≤0.01 µg/L correctly ruled out AMI with a sensitivity of 98.8% and a negative predictive value of 99.7%
