Predictive value of heart rate recovery and peak oxygen consumption for long-term mortality in patients with coronary heart disease

Aijaz B et al. – In pts with stable coronary heart disease (CHD) undergoing cardiac rehabilitation, the rate of heart rate (HR) recovery provides additional prognostic information beyond the peak oxygen consumption (peak VO2). An abnormal HR recovery identifies a subset of patients at intermediate risk despite a preserved peak VO2.
Methods- Study of relationship between poor exercise capacity and abnormal HR recovery during exercise testing in pts with CHD
- Cardiopulmonary treadmill testing at end of Phase II cardiac rehabilitation for 282 pts (17% women) with stable CHD
- Mean follow-up: 9.8 ± 2.9 yrs
- Cox proportional hazards regression with adjustment for low peak VO2, age, and gender to test prognostic significance of HR recovery
- Low peak VO2 defined as <19 mL/kg/min for men; <15mL/kg/min for women
- HR recovery defined as peak -- 1-minute after peak HR
- HR recovery and peak VO2 correlated mildly
- Both abnormal HR recovery (<13 beats/min) and low peak VO2 significantly associated with greater mortality in model of age, gender, low peak VO2, and abnormal HR recovery
- Despite preserved peak VO2, 10-yr mortality rate significantly greater in pts with HR recovery <13 vs ≥13 beats/min (13.6% vs 5.6%, respectively)
