Chen KY et al. – Findings show triple antiplatelet therapy as superior to dual antiplatelet therapy in pts with ST-segment elevation myocardial infarction undergoing primary percutaneous coronary intervention with drug-eluting stents.
Methods- Korean Acute Myocardial Infarction Registry (KAMIR): retrospective study of 4203 ST-segment elevation myocardial infarction pts who had primary percutaneous coronary intervention with drug-eluting stents
- Administration of either dual (aspirin + clopidogrel; dual group; n=2569) or triple (aspirin + clopidogrel + cilostazol; triple group; n=1634) antiplatelet therapy
- Additional cilostazol ≥1 mo for triple group
- Comparison of major adverse cardiac events at 8 mo between groups
- Similar incidence of major bleeding events but a significantly lower incidence of in-hospital mortality for triple vs dual group
- Clinical outcomes at 8 mo: significantly lower incidences of cardiac death, total death, and total major adverse cardiac events for triple vs dual group
- On subgroup analysis, more benefits for older (>65 yrs), female, and diabetic pts from triple vs dual antiplatelet therapy
