Subramaniam V et al. – A review reports that, overall, a reduction in blood pressure is still the most important factor in prevention of disease progression, and early treatment averts the risk of subsequent heart failure and stroke.
Methods- Review of pathophysiological spectrum relating blood pressure, drug treatments and stroke
- Left ventricular hypertrophy (LVH): most visible manifestation of target organ damage related to hypertension
- LVH predisposes to systolic/diastolic heart failure, associated with even greater risk of stroke
- Electrocardiography and echocardiography quantify LVH; cardiac MRI may be a more reproducible and accurate modality
- Regression in LVH associated with improved cardiovascular outcomes
- Treatments aimed at LVH regression: restriction of salt, regular exercise, weight reduction
- Renin–angiotensin–aldosterone system blockade is particularly effective in preventing cardiovascular and cerebrovascular events
- This blockade associated with LVH regression, decrease in left atrial size, and reduced incidence of new-onset atrial fibrillation, all possibly contributing towards a decreased vascular events
