Blaha M et al. – In select asymptomatic pts, absence of coronary artery calcium (CAC) predicts excellent survival with 10-yr event rates of ~1%. A finding of 0 CAC may serve as a rationale to emphasize lifestyle therapies rather than pharmacotherapy and to forgo repeat imaging studies. Pts with low CAC score (CAC 1-10) are at increased risk vs pts with a 0 score and may a distinct risk group.
Methods- Study to quantify mortality rates associated with absent and low positive (CAC 1-10) CAC
- Assessment of annualized all-cause mortality rates in 44,052 consecutive asymptomatic pts referred for CAC testing
- Mean follow-up: 5.6±2.6 yrs (range 1-13 yrs)
- On screening electron beam tomography: no CAC for 19,898 pts (45%); low levels (CAC 1 to 10) for 5,388 (12%); CAC >10 for 18,766 (43%)
- Mortality: 104 deaths for no CAC (0.52%) pts; 58 for CAC 1-10 (1.06%); 739 for CAC >10 (3.96%)
- Annualized all-cause mortality rates (deaths/1,000 person-yrs): 0.87% for CAC 0, 1.92% for CAC 1-10; 7.48% for CAC >10
- Hazard ratio (HR) for all-cause mortality among CAC 1-10 vs CAC 0 after adjustment for traditional risk factors: 1.99
- Smoking and diabetes mellitus associated with few events observed in CAC 0 group
