Impact of diabetes and gender on survival after acute myocardial infarction in the Province of Quebec, Canada—a population-based study

Ouhoummane N et al. – The higher 30-day mortality post-acute myocardial infarction (AMI) in younger (age 20-64 yrs) and middle-age (65-74 yrs) women vs men was not influenced by diabetes status. However, 5-yr follow-up showed similar gender mortality in pts without diabetes disappeared in younger diabetic pts, possibly due to deleterious, long-term, post-AMI impact of diabetes in younger women.
Methods- Study of the impact of diabetes, gender and their interaction on 30-day, 1-yr and 5-yr post- AMI mortality in 3 age groups (20-64, 65-74 and ≥75 yrs)
- Retrospective analysis of 23,700 pts age ≥20 yrs (22% with diabetes) admitted for first AMI April 1995-March 1997
- Review of administrative databases for pts and outcomes
- Non-significant interaction between diabetes and gender for 30-day mortality
- Independent of diabetes status, women age <75 yrs had ≥38% higher mortality vs men after adjustment for socioeconomic status and co-morbid conditions
- Controlling for in-hospital complications eliminated gender difference
- No significant gender disparity for all age groups for 1-yr mortality (31-365 days)
- During 5-year follow-up, no gender differences in any age group, except for younger (<65 yrs) women with diabetes, who had 52% higher mortality than men after controlling for co-variables
- This female disadvantage shown by significant interaction between diabetes and gender in pts age 65?yrs
