Does left atrial volume and pulmonary venous anatomy predict the outcome of catheter ablation of atrial fibrillation?

Hof I et al. – Left atrial (LA) volume is an independent predictor of atrial fibrillation (AF) recurrence after catheter ablation. Additionally, pulmonary venous (PV) anatomy had no effect on outcome. These findings suggest that assessment of LA volume be incorporated into preprocedural evaluation of pts considered for AF ablation.
Methods- Study of whether LA volume and PV anatomy, on computed tomography (CT) prior to ablation, predict AF recurrence following catheter ablation
- Assessment of 146 pts (mean age 57 ± 11 yrs, 83% male) with symptomatic AF: 55% paroxysmal, 18% persistent, 27% long-standing persistent
- CT scanning prior to catheter ablation for LA volume and PV anatomy
- Circumferential PV isolation guided by Cartomerge electroanatomical mapping
- Outcome defined as complete success, improvement, or failure
- At mean follow-up of 19 ±7 mo, complete success in 59 pts (40%) and improvement in 38 pts (26%)
- LA volume was independent predictor of AF recurrence
- PV variations equally distributed among different outcomes of ablation procedure
- Univariate analysis did not identify PV anatomy as a predictor of outcome
