Gao L-J et al. – Cutting balloon angioplasty (CBA) + bare metal stent (BMS) combination has a good long-term clinical effect for treatment of ostial left anterior descending artery (LAD) lesions and may serve as an alternative strategy for pts with a contraindication for drug-eluting stent (DES) implantation, pts who cannot tolerate long-term dual antiplatelet medication, or elderly pts.
Methods- Evaluation of whether CBA followed by BMS (CBA + BMS) for treating ostial LAD lesions is an alternative strategy
- Retrospective study of 101 consecutive pts (51 with DES and 50 with CBA + BMS) with ostial LAD stenting November 2003-May 2005
- Target vessel diameter: 3.0 mm
- DES group vs CBA + BMS group similar rates at 6-8 mo angiographic follow-up:
- Restenosis (10.3% vs 17.9%)
- Target lesion revascularization (TLR) (5.88% vs 10%)
- Major adverse cardiac events (MACE) (7.84% vs 12%)
- More bleeding events in DES group
- During 2-yr clinical follow-up, no myocardial infarction in the 2 groups
- Rates of TLR (7.84% vs 10%) and MACE (9.8% vs 12%) also similar
- MACE-free survival rate: 90.2% vs 88%
