Potena L et al. - Universal prophylaxis was associated with delayed onset of CMV infection, lower viral burden, reduced CMV disease/syndrome and less intimal thickening, as compared with a preemptive anti-CMV approach. Randomized studies are required to confirm the potential benefits of prophylaxis vs a preemptive approach in heart transplant recipients.

This study provide additional suggestive evidence that aggressive prevention of even low-grade and asymptomatic CMV infection may provide graft-related benefit in heart recipients. The interaction among anti-CMV approaches (i.e. prophylaxis vs. pre-emptive), different immunosuppressive regimens, and CMV specific immune-response, represent a crucial field of research in the aim to customize patients' drug regimen.
