Tomkin GH et al. – Although antidiabetic drugs affect lipid metabolism, little evidence suggests that these drugs can prevent atherosclerosis in diabetes; however, some antidiabetic drugs may promote atherosclerosis through an adverse effect on lipoproteins.
Methods- Literature review for pathways linking abnormalities in fatty acid metabolism, insulin resistance, and diabetes with cholesterol metabolism abnormalities in context of the effect of antidiabetic drugs on lipoprotein metabolism
- Postprandial lipoproteins are an established risk factor
- Both insulin resistance and diabetes are associated with abnormalities in chylomicron production, with identified clearance and regulatory genes
- Metformin has multiple actions affecting numerous genes
- Peroxisome proliferator-activated receptor-gamma regulation of insulin sensitivity and important effects on lipoproteins are reported
- The entero-insulin axis and glucagon-like peptide-1 agonists, with inhibitors of dipeptidyl peptidase 4 may have lipoprotein implications
