Evolution and outcome of diastolic dysfunction

Achong N et al. – Diastolic dysfunction (DD) is associated with all-cause mortality, independent of presence of a major co-morbidity; degree of DD remains stable in ~50% of pts, with more favorable outcome for pts whose diastolic function improves over time.
Methods- Cohort study to characterize influence of clinical features, medical therapy, and echocardiographic parameters on DD progression
- Review of 926 consecutive pts (62±14 yrs, 221 women) with DD and preserved systolic function
- Repeat echocardiography in 199 pts >1 yr after baseline study (average 3.6±1.4 yrs)
- Follow-up for 4.8±2.5 years: 97% complete for major endpoint of all-cause mortality
- At follow-up, 142 pts died and 22 admitted with heart failure
- Independent predictors of death: age, hyperlipidemia, co-morbid disease, restrictive filling
- DD degree stable in 52%, deteriorated in 27% and improved in 21%
- More use of medical therapy in pts with stable or worsening diastolic function
- On multivariate analysis considering medical therapy use, improved DD associated with survival benefit
