Vasireddi SR et al. – A 73–year–old black man with a history of schizoaffective disorder (bipolar type, dementia–not otherwise specified) was admitted a few years ago. In addition to psychiatric problems, he had hypertension, intermittent arrhythmia (bradycardia), chronic renal failure, and hypothyroidism. His most recent thyroid–stimulating hormone (TSH) test was within normal limits. He had been experiencing constipation, hyperuricemia, had a positive purified protein derivative (PPD) test, anemia, and had gastroesophageal reflux disease.
